The IRS sent 6,000 public libraries incorrect 1040X forms and instructions. The incorrect forms bear the revision date, "REV. December 2006." The correct forms have a revision date of February 2007. Dr. Steve Groskin will join Dr. Joe England and Nurse Practitioner Kathy Westall in the Jamestown Family Practice beginning in April. Isabel Perry Young, 91, formerly of Jamestown passed away peacefully in Las Vegas, Nev., on Jan. 27, 2007. She was the wife of the late Joe Young and the widow of the late Robert Howe.
